Need a copy of your discharge summary, reports, or complete case file? Our MRD maintains every patient\u2019s records safely and confidentially — and provides certified copies through a simple, transparent process.
Duplicate copy of your discharge summary with diagnosis, treatment given and follow-up advice.
Copies of blood tests, pathology, biopsy and other investigation reports from your treatment.
X-ray, ultrasound, CT, MRI and PET-CT reports — films/CDs where applicable.
Surgical procedure notes and anaesthesia records for surgeries performed at our hospital.
Certified photocopy of the entire indoor case file — for insurance, legal or referral purposes.
Duplicate bills and payment receipts for insurance claims and reimbursement.
Visit the MRD counter and fill the records request form with patient details and UHID/IPD number.
Patient\u2019s ID proof — or requester\u2019s ID with relationship proof/authorisation if applying on behalf.
Pay the copying charges at the counter and collect your acknowledgment slip with collection date.
Collect certified copies on the given date — typically within 72 hours of request.

Yes. Visit the MRD counter with the patient\u2019s ID proof and UHID/IPD number (or approximate admission dates if you don\u2019t have the number). Fill the request form, pay the nominal copying fee, and collect your certified duplicate — usually within 72 hours.
Most requests are ready within 72 working hours. Complete case file copies for long admissions may take a few days longer due to page volume. Urgent requirements (ongoing treatment elsewhere, insurance deadlines) — mention this at the counter and we will try to expedite.
Yes — they must carry their own photo ID plus a written authorisation letter signed by you (the patient), along with a copy of your ID. For minor patients, parents/guardians can apply directly. For deceased patients, the legal heir applies with heirship proof.
Typically insurers need: discharge summary, final bill with payment receipts, investigation reports, and sometimes the complete indoor case file. Check your insurer\u2019s exact list, then request all of them together in one application to save time. TPAs can also apply directly with your signed consent.
We retain records as per government regulations and NABH norms — indoor records are typically preserved for many years (and permanently for medico-legal cases). Even for treatment taken years ago, contact MRD with the patient name and approximate dates; our team will search the archives.
No. Records are released only to the patient, their authorised representatives, or under legal mandate (court/police requisition). Insurance companies and TPAs receive records only with your signed consent. Every release is documented.
Only nominal copying/processing charges apply — far below private hospital rates, in line with our charitable mission. Exact charges depend on the number of pages and whether films/CDs are needed. The counter will tell you the amount before you pay.
